- Should be registered with Medical Council of India / Medical Council of State.

- Candidates intending to join as Contract Medical Practitioner should apply on prescribed format and submit the same up to 10.30 A.M on the date of interview at the venue along with self attested photocopies of following documents and attend the “Walk-in-Interview” with original certificates mentioned below.

- 04 (four) copies of recent passport size photograph with self signature.
- Original Pension Payment Order book & self attested copy of the same in case of Retd. Doctor.
